American Specialty Health@ Networks Affinity Program

PacifiCare has a vendor arrangement with American Specialty Health Networks (ASH Networks), whose Affinity program offers PacifiCare enrollees discounts of up to 25 percent on the following:

• Acupuncturists
• Chiropractors
• Massage Therapists
• Dietitians

The program is convenient and easy to use:

1. Select a participating complementary health care provider from the ASH Networks directory, or call PacifiCare Customer Service.
2. Select a participating ASH Networks provider online by linking to the Healthy Roads site through ASHN.com
3. Call the ASH Networks provider to verify participation in the network and to schedule an appointment for examination. No physician referral is required.

Please keep in mind:

• Your ASH Networks provider will develop, if necessary, a treatment plan for you.
• You may change participating ASH Networks providers at any time.
• With the Affinity program, you simply pay the enrollee fees directly to the ASH Networks provider.
• There are no deductibles or annual limits to this program. Remember that it's important to communicate with your Primary Care Physician about any complementary health care treatment you are receiving as well as any dietary supplements you are taking
