Dental Administrators, multiple plan designs enable
us to meet any group's requirements and even individuals,
too: Dental HMO, Dental PPO, Dental Indemnity and Individual
Dental HMO.
Our plans are supported by some of the largest dental
health maintenance organization (DHMO) networks and
preferred provider organization (PPO) networks. And
because provider experience is critical to customer
satisfaction, we financially reward those dentists in
our dental HMO* network who deliver quality service
to our members.

VISION
Vision PPO
PacifiCare Dental and Vision Administrators offer two
vision PPO products: the Eyewear Only Plan and the Full
Service Plan. As PPO plans**, they offer the flexibility
of both in-network and out-of-network benefits. Additionally,
each plan type offers two different coverage levels
to suit any budget: Basic and Plus. Currently, vision
plans are only available to employer groups of 2 or
more.

What makes
PacifiCare Vision Plans Unique?
PacifiCare's Eyewear Only Plan is one of the few "materials-only"
plans on the market today. Our vision sales leader,
the plan helps employers cut redundant health care costs
while helping employees enjoy substantial eyewear savings.
With this plan, exams are covered through the employer's
medical plan.

PacifiCare Dental (a California-licensed HMO dental
plan) and PacifiCare Dental & Vision Administrators
(third-party administrator of indemnity and PPO dental
and PPO vision plans) provide HMO, PPO, and fee-for-service
dental and PPO vision benefits directly to individuals
and employer groups, and to seniors through Secure Horizons.
